ASD/ADHD Referral Information

Right to Choose

If you are based in England under the NHS you now have a legal right to choose your mental healthcare provider and your choice of mental healthcare team. This important right means that, for instance, should you decide the waiting time for your ADHD assessment is too long, then you can choose alternative providers.  The provider must supply the service to the NHS somewhere in England.

See the NHS website for more information regarding Mental Health Services Right to Choose.

How do I get referred?

We understand you would like to be referred for Adult ADHD/ASD assessment under the right to choose.

We will need to know which provider you wish to be referred to before we can begin the process, so please do your research and find a provider that is right for you prior to contacting us for a referral.

Once you have found the provider you want your referral to go to, you can find the self assessment form on their website. Download this form, and once you have completed it you can bring it into the surgery for the attention of the practice secretary, or your usual GP.

Once received, we will complete the referral and send you confirmation.

Please note, that if you require medication after diagnosis, we do not undertake shared care agreements with any of the ASD/ADHD services.
