Change of Address Notification

Allows you to submit change of address information online. Please note that a current mobile phone number is required to complete the online process as the practice will require verification of the new address to be sent prior to the GP patient record being updated. Be aware that if your new address is not within the practice area, you will be asked to register with a practice that serves the area you have moved to. You can check if the address is within our practice area.

Please note that if you do not have a current mobile phone number, you will be unable to process your change of address using this form. You can download a copy of the change of address form at , print out and complete the form and post it to the practice, together with a copy of the new address verification official document, or put it through the practice letterbox

What Happens Next

Once we receive your notification, we will send a SMS message to your mobile phone, requesting a photograph of an official document verifying your new address. We are unable to process any change of address until you have returned the official document electronically. If you are unable to send a photo of the official document electronically, you will need to take a copy of the official document and post it through the practice letterbox.Official documents may include a copy of a recent utility bill or other official correspondence (Bank statement, Council Tax etc)
